Final Fantasy XV Error CE-34878-0

I see a lot of people saying "It happens because of X", maybe that's how it manifests in your version of the game, but I think it's fair to say that we're seeing a range of things triggering it. I've had it happen on autosave, had it happen twice when I paused the game last night, had it happen while driving a few times, happen mid-battle (or right at the end of a battle) frequently, had it happen on quest completion, and on manual saves. Sometimes it happens randomly while running through the countryside. As yet I've seen no discernable pattern within my version of the game.

The only constant is that when it kicks me out the game, it also disconnects the PS4 from the PSN.
