Does it happen with any other game or just FIFA 16?
There could possibly be an issue with the power supply, but before going down that route, run option 5 from the safe menu (rebuild database) then delete the game data and reinstall.
It is worth also trying a different power lead if at all possible.
If that doesn't help, you could use the online service tool to organise repair from Sony > https://hardware.support.eu.playstation.com/en_GB#/
Alternatively, if you are confident in your skill level, you could try to isolate the issue yourself but that obviously is at your own risk. Replacement parts are out there, but will be from faulty units that have been broken for parts.
