I have a paragon lvl 214 demon hunter that i ahve beaten the entire story mode with, TWICE. he currently has over 500 million gold, and almost all of his gear has gems socketed into them. With that character, and the two that I have made since then, I have completed over 700 bounties. Yet I have still not gotten most of the trophies that I should have been awarded at this point. tux503 is right, unfortunately. Play Station will probably never get around to fixing this, as they have no need to. however, getting achievements honestly shouldn't affect the overall gaming experience that much.
